Medical Records

What We Do

The Medical Records Department is the “final resting place” of all the test reports, doctor’s notes, and all other documents related to your health care visits. Here it is checked for accuracy, put in order and filed safely away under your Medical Record number.

The Medical Records Department is also responsible for coding your visits in preparation for billing the insurance company. “Coding” involves assigning numeric codes for the diagnoses and procedures provided by the doctors. The insurance companies then pick these codes up in preparation to paying the hospital.

Transcription is done in our department also. Most providers dictate their notes on cassette tapes which then need to be typed out. While most of our transcription is outsourced, some is done here.

Medical Records handles your correspondence requesting records.

Where We're Located

The Medical Records Department is located in the Orin Flin building next to the hospital. If you come in through the sliding glass doors leading to the clinic, you will see an elevator in front of you. Take the elevator down to “M” for Medical Records. Turn left and follow the corridor to our department.

Frequently Asked Questions

How can I get my health records for my own use?

Original healthcare records belong to the facility generating them. You must sign a “Release of Information” in order to get copies of your records.

How can I get a Release of Information?

You can come down to the department and we will be happy to provide one or you can call and we can fax or mail you one.

How long will it take to get my records?

State law allows us 10 business days to provide the records. We make every effort to get them to you sooner.

Is there a cost to this?

We are allowed by State law to charge 75 cents per page. It is our policy to only charge this amount if you want 10 pages or more.

What if my provider or insurance company wants my records?

If your provider is a Bassett provider, no Release of Information is necessary. If he is not, we will require a Release. No release is necessary for most insurance requests.

We will fax your records to your provider. We mail records to insurance companies and other non-healthcare entities.

The Medical Record can be shared only with permission of the patient.

This is confidential information and protected from disclosure and will not be released except to the person/institution indicated on the form.
